Notice of Data Security Incident

CEI Vision Partners, LLC (“CVP”) operates a network of eye care practices, including Cincinnati Eye Institute. CVP is notifying individuals of a data security incident.

On May 26, 2024, CVP discovered that an unknown, unauthorized third party gained temporary access to CVP’s computer network. Upon discovery, we promptly secured the network from further unauthorized access and began an internal investigation. We also engaged a forensic security firm to further investigate and confirm the security of our computer systems. The forensic investigation determined that the unauthorized party accessed certain CVP computer systems between May 24, 2024, and May 27, 2024. We have undertaken a thorough review of the relevant records that required considerable time to ensure accuracy and completeness. We have recently completed this review.

On June 10, 2025, CVP determined that the files contained some individuals’ personal information. The information varied for each individual but may have included your name and contact information, together with your date of birth, Social Security number, financial account information, driver’s license number, limited clinical information, and/or health plan information.

CVP is not aware of any fraud or identify theft to any individual as a result of this incident. Nonetheless, CVP recommends that, as a precautionary measure, all individuals should remain vigilant to protect against potential fraud and/or identity theft by, among other things, reviewing their account statements and monitoring credit reports closely. If individuals detect any suspicious activity on an account, they should promptly notify the financial institution or company with which the account is maintained and report any fraudulent activity or any suspected incidents of identity theft to proper law enforcement authorities, including the police and their state’s attorney general. Individuals may also wish to review the tips provided by the Federal Trade Commission (“FTC”) on fraud alerts, security/credit freezes and steps that they can take to avoid identity theft.
